Want some good examples for given situations

Recent and local examples are preferred, but international ones are also welcome.

1. New product failure due to Overestimated market size
2. New product failure due to Poor design
3. New product failure due to Priced too high
4. New product failure due to Ineffective promotion
5. New product failure due to Management influence
6. New product failure due to High development cost

I will really appreciate good example/s for the above situations, no worry if you can't give example for every point, even one, two points are good however more will be preferred and please sooner the better.

I know a person that once manufactured an artificial sweetener for profit here in Pak. He really struggled with the design, as that product is mostly used by seniors - Old people were facing difficulty in taking out the pill because it required a good shake or whack. He changed that poor design only to discover that the new design also failed miserably because now the pills were very easy to extract and the people used to end up using more pills than needed in their drinks. Hence, the manufacturers bear high development cost (mostly in the design phase) with poor design also their price was 'now' too high as compared to their competitors, also the competitors exploited economies of scale and other strategies - such as decreasing the quality of the product - to further reduce their price. So, this example pretty much covers 3 of your points.
